Employee Resources for COVID-19 Vaccine Requirement

In compliance with Executive Order (EO) 14042, signed by President Biden in September, the Olgoonik family of companies is requiring all employees (regardless of work location) to be fully vaccinated against COVID-19 by Dec. 8, 2021. Employees must provide proof of vaccination by the deadline or have an approved medical or religious accommodation on file with Human Resources (HR).

To document compliance, all employees must submit their vaccination record to HR by Dec. 8, 2021, or an accommodation request by Nov. 19, 2021 (submission does not guarantee approval). Any employees who do not comply with this federal mandate (whether through proof of vaccination or with an approved accommodation) by the stated deadline will no longer meet the requirements for employment and may be placed on an unpaid leave status or separated from employment.

1. Visit https://ess.olgoonik.com.
2. Click on HR Actions in the lower left corner.
3. Click on Start an Action that Applies to Me
4. Select Document Upload Form from the drop down menu
5. Click Submit
6. Use today’s date for the effective date
7. Attach your file
8. Sign and Date
9. Click Initiate Action
